SERVICES

Prime Weight Loss & Wellness provides doctor-supervised medical weight loss services, including FDA-approved and compounded prescription medications, nutritional counseling, and telehealth consultations. Services are provided via licensed providers and a partner pharmacy and may not be available in all states.

TEXT MESSAGING TERMS

You must be 18 years or older to use our SMS messaging service.

By opting in to receive text messages from Prime Weight Loss & Wellness, you agree to the following:

  •  You can cancel the SMS service at any time. Just text “STOP” to (602) 835-8655. After you send the STOP message, we will send you a reply message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us.
  •  If you are experiencing issues with the messaging program, you can reply with the keyword “HELP” for more assistance, or you can get help directly at (602) 835-8655.
  •  Carriers are not liable for delayed or undelivered messages.
  •   As always, message and data rates may apply for any messages sent to you from us and to us from you. You will receive appointment-related messages and, if you have opted in, marketing messages. Message frequency varies based on your interaction with our services.

APPOINTMENT POLICY

Appointments and consultations are scheduled through our online booking system. Cancellations must be made at least 24 hours in advance. Patients are responsible for providing accurate health information and attending scheduled appointments.

MEDICAL DISCLAIMER

All medical weight loss programs are supervised by licensed medical professionals. Individual results may vary. A medical consultation and intake review is required before beginning any treatment program. Eligibility for prescription medications is determined by the licensed provider based on medical history, current health status, and applicable state laws.

LIMITATION OF LIABILITY

Prime Weight Loss & Wellness provides information and services in good faith. We are not liable for any indirect, incidental, or consequential damages arising from the use of our services or communications.
